Can Peyronie’s disease go away on its own?

In cases where the curvature is significant enough to interfere with sexual activity or cause psychological distress, surgical treatment is often the most effective and lasting solution.

Plaque incision and grafting is recommended for men with curvature greater than 60 degrees and preserved erectile function. This procedure involves releasing the scar tissue (plaque) and placing a graft to restore straightness while maintaining penile length. It requires specific expertise and is typically reserved for more complex cases.

Penile prosthesis implantation is the preferred option when severe curvature is accompanied by erectile dysfunction. A malleable or inflatable implant is inserted to restore rigidity, and the curvature is corrected either through manual straightening techniques or additional grafting during the same procedure.

Tunical plication procedures (such as the Nesbit technique) are an option for men with less severe curvature who have good erectile function. This involves shortening the longer side of the penis to achieve straightening. It is effective but may result in some loss of length although this can be minimised.

  • Injection therapy: intralesional injections such as verapamil or collagenase clostridium histolyticum (where available) may help soften plaque and improve curvature
  • Oral medications and supplements: some are prescribed in early stages, but evidence for their effectiveness is limited
  • Low-intensity shockwave therapy (LiSWT): may help relieve pain in the acute phase, though it does not correct curvature

In the stable (chronic) phase, when the curve has stopped progressing, surgery is the most effective option for men with significant deformity or sexual difficulty:

Surgical treatments:

  • Plication surgery: suitable for mild to moderate curvature and good erectile function; the longer side of the penis is shortened to achieve straightening
