What Is Penile Reconstruction?
Penile reconstruction, also referred to as phalloplasty or penile salvage surgery in certain contexts, involves rebuilding the penile structure to restore appearance and function. The procedure can vary significantly depending on the underlying cause and the extent of tissue loss or deformity.
Some of the most common reasons for penile reconstruction include:
- Penile trauma (e.g. accidents, burns, bites, self-injury)
- Cancer treatment, such as partial or total penectomy
- Severe infection (e.g. Fournier’s gangrene)
- Complications from previous surgeries, including prosthetic or aesthetic procedures
- Congenital conditions, such as hypospadias or buried penis
- Gender affirming surgery
Tailored Techniques for Individual Needs
No two reconstructions are the same. My approach is always tailored to the patient’s anatomy, goals, and health status. Techniques may include:
- Local tissue flaps: Using remaining penile or scrotal skin to rebuild shaft contours or restore coverage.
- Skin grafts or flap reconstructions: For more extensive defects, we may use tissue from the thigh, forearm, or abdomen to reconstruct the penis with sufficient bulk, skin coverage, and potential for prosthetic implantation later on.
- Urethral reconstruction: When the urinary channel is affected, reconstructing a functional urethra is essential to allow standing voiding.
In certain cases, penile reconstruction can also include or prepare for the placement of a penile prosthesis, enabling sexual function following healing.
