Erectile dysfunction (ED) is a condition that affects millions of men worldwide, and for some, conservative treatments such as medication or injections do not provide the relief they need. In such cases, a penile prosthesis or penile implant can be a life-changing solution. As a consultant urologist specialising in complex penile reconstruction and sex-affirming genital surgery, I have had the privilege of guiding many patients through this process with care, precision and discretion.

What Is a Penile Prosthesis?

A penile prosthesis is a medical device surgically implanted in the penis to enable an erection sufficient for sexual intercourse. It is usually recommended for patients with severe ED who have been unsuccessful with other therapies. These devices come in two main types:

  • Inflatable prostheses: These devices are the most commonly used today and consist of cylinders in the penis, a fluid reservoir in the abdomen and a pump in the scrotum. When the pump is activated, fluid is pumped to the cylinders, creating a firm erection. The penis can deflate again after intercourse by reversing the process.
  • Malleable (Semi-Rigid) Prostheses: These consist of bendable rods that keep the penis firm but flexible. They are mechanically simpler and may be preferred in some cases, such as patients with limited mobility or certain anatomical conditions.

Why Choose a Penile Implant?

For many men, a penile prosthesis is more than a treatment – it is a chance to regain their confidence, intimacy and quality of life. Unlike drugs, an implant offers spontaneity, reliability and longevity. It does not affect sensation or the ability to achieve orgasm, and partner satisfaction is also high because expectations are clearly managed.

As with any surgical procedure, a thorough evaluation is crucial. My approach combines careful advice, individualised planning and evidence-based surgical techniques to ensure patients feel informed and supported every step of the way.

Surgical Expertise Matters

Penile prosthesis surgery is highly specialised and should be performed by experienced surgeons familiar with both standard and complex cases. I often see patients with anatomical variations, previous surgeries or concurrent conditions such as Peyronie’s disease, which require a more nuanced approach.

In addition, I often care for transmasculine patients undergoing sex-affirming phalloplasty, where penile prostheses play a crucial role in the functional outcome. In these cases, surgical planning is even more tailored, taking into account vascular supply, soft tissue coverage and urethral reconstruction.

What to Expect: Before, During, and After

The procedure begins with a comprehensive consultation. We discuss the pros and cons of each prosthesis type, surgical risks and the recovery process. My team ensures that patients have realistic expectations and are mentally and emotionally prepared.

Surgery usually takes 1 to 2 hours under general or spinal anaesthesia, and most patients go home the same or next day. Recovery includes a short period of limited activity and patients are usually able to resume sexual activity within 6 to 8 weeks.

Importantly, my follow-up protocol is comprehensive. It includes early assessment, activation education and ongoing support to optimise satisfaction and functioning.

The Bottom Line

Penile prostheses are not just medical devices. They are tools that help restore intimacy, autonomy and self-esteem. As someone highly dedicated to urological reconstruction and sexual medicine, I take pride in providing this solution with the empathy and expertise each patient deserves.
